At Kuljic DDS & Team: Center for Integrative Dentistry, we believe you should never have to choose between the very best treatment available and something that just fits your budget. That’s why we work hard to make payment for treatment easy and affordable for you and your family.

Additionally, we are happy to estimate fees and insurance reimbursement before beginning your treatments, and as a courtesy we will gladly file insurance forms for you. Optimal, comprehensive dentistry is what we practice, and we do not base our treatment recommendations on what your insurance will or will not pay.

As an out-of-network provider, our recommendations are based on what is best for you. Any treatment you choose to receive that is not covered by your insurance is your financial responsibility at the time of treatment unless other arrangements have been made. If you have any questions, please ask; it’s our pleasure to help however we can!

Claims will be submitted at the time of service as “Benefits to Patient,” and for covered services, reimbursements will be sent directly to you, the patient, from your insurance carrier. Patients make payment arrangements in office prior to beginning treatment and pay the office directly.

If you have dental insurance, your dental plan may pay towards some services (we will submit to medical first and whatever medical doesn’t cover, can be submitted to dental. Remodeling is the description that the orthodontic services fall under as they are part of the medically necessary arch expansion claim that is sent to medical insurance). We can send it to your dental insurance (if applicable) after medical processes the claim.

Essentially, you pay the office, and your insurances would pay you directly for covered services after any deductible is met.
